ABSTRACT

Objective: To investigate the clinical features and short-term prognosis of patients with SARS-CoV-2 infection associated acute encephalopathy (AE). Methods: Retrospective cohort study. The clinical data, radiological features and short-term follow-up of 22 cases diagnosed with SARS-CoV-2 infection associated AE in the Department of Neurology, Beijing Children's Hospital from December 2022 to January 2023 were retrospectively analyzed. The patients were divided into cytokine storm group, excitotoxic brain damage group and unclassified encephalopathy group according to the the clinicopathological features and the imaging features. The clinical characteristics of each group were analyzed descriptively. Patients were divided into good prognosis group (≤2 scores) and poor prognosis group (>2 scores) based on the modified Rankin scale (mRS) score of the last follow-up. Fisher exact test or Mann-Whitney U test was used to compare the two groups. Results: A total of 22 cases (12 females, 10 males) were included. The age of onset was 3.3 (1.7, 8.6) years. There were 11 cases (50%) with abnormal medical history, and 4 cases with abnormal family history. All the enrolled patients had fever as the initial clinical symptom, and 21 cases (95%) developed neurological symptoms within 24 hours after fever. The onset of neurological symptoms included convulsions (17 cases) and disturbance of consciousness (5 cases). There were 22 cases of encephalopathy, 20 cases of convulsions, 14 cases of speech disorders, 8 cases of involuntary movements and 3 cases of ataxia during the course of the disease. Clinical classification included 3 cases in the cytokine storm group, all with acute necrotizing encephalopathy (ANE); 9 cases in the excitotoxicity group, 8 cases with acute encephalopathy with biphasic seizures and late reduced diffusion (AESD) and 1 case with hemiconvulsion-hemiplegia syndrome; and 10 cases of unclassified encephalopathy. Laboratory studies revealed elevated glutathione transaminase in 9 cases, elevated glutamic alanine transaminase in 4 cases, elevated blood glucose in 3 cases, and elevated D-dimer in 3 cases. Serum ferritin was elevated in 3 of 5 cases, serum and cerebrospinal fluid (CSF) neurofilament light chain protein was elevated in 5 of 9 cases, serum cytokines were elevated in 7 of 18 cases, and CSF cytokines were elevated in 7 of 8 cases. Cranial imaging abnormalities were noted in 18 cases, including bilateral symmetric lesions in 3 ANE cases and "bright tree appearance" in 8 AESD cases. All 22 cases received symptomatic treatment and immunotherapy (intravenous immunoglobulin or glucocorticosteroids), and 1 ANE patient received tocilizumab. The follow-up time was 50 (43, 53) d, and 10 patients had a good prognosis and 12 patients had a poor prognosis. No statistically significant differences were found between the two groups in terms of epidemiology, clinical manifestations, biochemical indices, and duration of illness to initiate immunotherapy (all P>0.05). Conclusions: SARS-CoV-2 infection is also a major cause of AE. AESD and ANE are the common AE syndromes. Therefore, it is crucial to identify AE patients with fever, convulsions, and impaired consciousness, and apply aggressive therapy as early as possible.

ABSTRACT

Objective To analyze the epidemiological characteristics of community transmission of the coronavirus disease 2019 (COVID-19) caused by four imported cases in Hebei Province, and to provide a scientific basis for the prevention and control of the disease. Methods Descriptive epidemiological methods were used to analyze the epidemiological characteristics of four community-transmitted COVID-19 outbreaks reported in the China Disease Control and Prevention Information System from January 1, 2020 to December 31, 2021 in Hebei Province. Results From January 1, 2020 to December 31, 2021, four community-transmitted COVID-19 outbreaks caused by imported COVID-19 occurred in Hebei Province, respectively related of Hubei (Wuhan) Province, Beijing Xinfadi market, Overseas cases and Ejina banner of Inner Mongolia Autonomous Region. Total of 1 656 cases (1 420 confirmed cases and 236 asymptomatic cases) were reported, including 375 cases in phase A (From January 22 to April 16, 2020), and phase B (from June 14 to June 24, 2020) 27 cases were reported, with 1 116 cases reported in the third phase (Phase C, January 2 to February 14, 2021), and 138 cases reported in the fourth phase (Phase D, October 23 to November 14, 2021). The 1 656 cases were distributed in 104 counties of 11 districts (100.00%), accounting for 60.46% of the total number of counties in the province. There were 743 male cases and 913 female cases, with a male to female ratio of 0.81:1. The minimum age was 13 days, the maximum age was 94 years old, and the average age (median) was 40.3 years old. The incidence was 64.01% between 30 and 70 years old. Farmers and students accounted for 54.41% and 14.73% of the total cases respectively. Of the 1 420 confirmed cases, 312 were mild cases, accounting for 21.97%;Common type 1 095 cases (77.11%);There was 1 severe case and 12 critical cases, accounting for 0.07% and 0.85%, respectively. 7 patients died from 61.0 to 85.7 years old. The mean (median) time from onset to diagnosis was 1.9 days (0-31 days), and the mean (median) time of hospital stay was 15 days (1.5-56 days). Conclusions Four times in Hebei province COVID-19 outbreak in scale, duration, population, epidemic and type of input source, there are some certain difference, but there are some common characteristics, such as the outbreak occurs mainly during the legal holidays or after starting and spreading epidemic area is mainly in rural areas, aggregation epidemic is the main mode of transmission, etc. To this end, special efforts should be made to strengthen the management of people moving around during holidays, and strengthen the implementation of epidemic prevention and control measures in places with high concentration of people. To prevent the spread of the epidemic, we will step up surveillance in rural areas, farmers' markets, medical workers and other key areas and groups, and ensure early detection and timely response.Copyright © 2022 China Tropical Medicine. ABSTRACT

Traditional Chinese medicine (TCM) has significant benefits in the prevention and treatment of diseases due to its unique theoretical system and research techniques. However, there are still key issues to be resolved in the full interpretation and use of TCM, such as vague active compounds and mechanism of action. Therefore, it is promising to promote the research on TCM through innovative strategies and advanced cutting-edge technologies. Microfluidic chips have provided controllable unique platforms for biomedical applications in TCM research with flexible composition and large-scale integration. In this review, the analysis and biomedical applications of microfluidics in the field of TCM are highlighted, including quality control of Chinese herbal medicines (CHMs), delivery of CHMs, evaluation of pharmacological activity as well as disease diagnosis. Finally, potential challenges and prospects of existing microfluidic technologies in the inheritance and innovation of TCM are discussed.Copyright © 2022 Elsevier B.V.

ABSTRACT

With the rapid development of China's economy, enterprises need to plan their logistics transportation routes reasonably in advance. This will make the transportation service more efficient. For the supplier's transportation service problem, an analysis method of critical path nodes is provided and a multi-supplier collaborative transportation strategy is designed in this article. First, a model for minimising the transportation cost was established, then a path diagram was simulated and the optimal and alternative transportation paths of suppliers based on the k-shortest path algorithm were calculated. In addition, path node availability during COVID-19 is used as a research context in this article. A multi-stage path analysis method was provided by discussing different cases of critical path nodes, which can make a reasonable selection of paths in a timely and effective manner. Finally, simulations of collaborative transportation for suppliers were performed in three scenarios and the results verified the effectiveness of the collaborative transportation strategy. The proposed collaborative transportation strategy of suppliers not only strengthened the synergistic cooperation among suppliers, but also cultivated the potential customer for suppliers in this article. Furthermore, the strategy could improve the flexibility of the supply chain, maximise the overall efficiency and also provide a new solution for the development of logistics and transportation services.

ABSTRACT

Objectives: This study aimed to describe the pathogen spectrum of bacteria and viruses of RTIs in hospitalized children during the Coronavirus disease 2019 (COVID-19) epidemic in Shenzhen. Method(s): From October 2020 to October 2021, the results of pathogenic tests causing RTIs were retrospectively analyzed in hospitalized children in Shenzhen Luohu Hospital Group. Result(s): 829 sputum samples for bacterial isolation and 1,037 nasopharyngeal swabs for virus detection in total. The positive detection rate (PDR) of bacteria was 42.1%. Staphylococcus aureus (18.8%) was the predominant bacteria detected in positive cases, with Moraxella catarrhalis (10.9%), Streptococcus pneumoniae (9.5%) following. The PDR of the virus was 65.6%. The viruses ranking first to third were Human Rhinovirus (HRV), Respiratory syncytial virus (RSV), and Human Parainfluenza (HPIV), with rates of 28.0, 18.1, and 13.5%, respectively. Children under 3 years were the most susceptible population to RTIs. The pathogens of S. aureus, M. catarrhalis, S. pneumoniae, HRV, and HPIV were more prevalent in autumn. Meanwhile, RSV had a high rate of infection in summer and autumn. S. aureus and HRV had higher co-infection rates. Conclusion(s): Our findings demonstrate the pathogen spectrum of 1,046 hospitalized children with RTIs in Shenzhen, China, during the COVID-19 outbreak.Copyright © 2022 the author(s), published by De Gruyter, Berlin/Boston.

ABSTRACT

Objective This multicenter clinical evaluation analyzed the clinical performance of five fast nucleic acid detection systems for 2019-nCoV. Methods Clinical performance of the five fast nucleic acid detection reagents approved in China was evaluated in the present study. Fifty-seven throat swabs samples from COVID-19 patients and fifteen throat swabs samples from healthy people were collected from the First Affiliated Hospital of Zhejiang University school of Medicine, Tongji Hospital of Tongji Medical College of HUST, and National Institute of Viral Disease Control and Prevention of CDC to evaluate the positive coincidence rate, negative coincidence rate, total coincidence rate, the detection time and retest rate as well as the relation between positive intensity and positive coincidence rate of the five fast nucleic acid detection systems in November 2020. Results The positive coincidence rates of the five kits were 92.59% (50/54), 83.64% (46/55), 98.25% (56/57), 94.44% (51/54) and 98.18% (54/55);and the negative coincidence rates were 93.33% (14/15), 93.33% (14/15), 86.67% (13/15), 100% (14/14) and 93.33% (14/15);and the total coincidence rates were 92.75% (64/69), 85.71% (60/70), 95.83% (69/72), 94.20% (65/69) and 97.14% (68/70), respectively. The positive coincidence rate of the five kits reached 100% for the strong-positive (90/90) and medium-positive samples (84/84), but only 82.18% (83/101) for weak-positive samples (cycle threshold value>33), and the retest rate of two kits were 15.28% (11/72) and 12.50% (9/72), which were both higher than 10%. Total time from sample extraction to amplification was between 32.33-65.33 minutes for these five kits. Conclusion The five fast nucleic acid detection reagents have good performance and can be used as a supplement to routine nucleic acid detection reagents.Copyright © 2022 Chinese Journal of Laboratory Medicine. ABSTRACT

Introduction: COPD patients are considered at risk of severe COVID-19 illness, however there are limited data on the burden of COVID-19 disease in this patient population. This study investigated the rate of COVID-19 diagnosis and hospitalization for COPD due to COVID-19 (pre-vaccination) over time, stratified by disease severity. Method(s): A retrospective cohort of COPD patients aged >=35 years, FEV1/FVC score <0.7, indexed from Mar-Aug 2020, using the English Clinical Practice Research Datalink Aurum database and Hospital Episode Statistics datasets. Monthly incidence rates of COVID-19 diagnosis and overall inpatient hospitalizations were described for all patients and by GOLD 2019 disease grade. Result(s): The study identified 103,105 COPD patients (mean [SD] age: 69.6 [10.6] years, 54.1% males) with 42.0%, 25.9%, 4.5%, and 6.3% in GOLD A, GOLD B, GOLD C, and GOLD D disease grade groups respectively, in the 12 months prior to index. Results over time are shown in Figure 1. Incidence rates of COVID-19 hospitalization (95% CI) per 100,000 person-days for the Mar-Aug period were GOLD A (2.2 [1.9, 2.6]), GOLD B (4.7 [4.1, 5.4]), GOLD C (4.6 [3.2, 6.3]), and GOLD D (7.9 [6.3, 9.7]). Conclusion(s): Among COPD patients, COVID-19 incidence peaked in April 2020. COVID-19 hospitalization rates were significantly higher in patients with more severe COPD and highest in GOLD D group patients. (Figure Presented).

ABSTRACT

To support building operations in reaching ultra-low energy targets, this paper proposes a data-informed building energy management (DiBEM) framework to improve energy efficiency systematically and continuously at the operation stage. Specifically, it has two key features including data-informed energy-saving potential identification and data-driven model-based energy savings evaluation. The paper demonstrates the proposed DiBEM with a detailed case study of an office and living laboratory building located in Cambridge, Massachusetts called HouseZero. It focuses on revealing the performance of the energy-efficient interventions from two-years' building performance monitoring data, as well as evaluating energy savings from the interventions based on the data-driven approach. With Year 1 as baseline, several interventions are proposed for Year 2 including improvements to controls and operation settings, encouragement of occupants' behavior for energy savings, and hardware retrofitting. These were deployed to heating/cooling, domestic hot water, lighting, plug and other loads, and photovoltaic (PV) systems. To quantify the impacts of different interventions on energy end uses, several data-driven models are developed. These models utilize linear regression, condition model, and machine learning techniques. Consequently, the heating/cooling energy consumption that was already ultra-low in Year 1 (12.8 kWh/m2) is further reduced to 9.7 kWh/m2 in Year 2, while the indoor thermal environment is well maintained. The domestic hot water energy is reduced from 2.3 kWh/m2 to 1.2 kWh/m2. The lighting energy is only increased from 0.3 kWh/m2 in pandemic operations without occupancy in Year 1 to 0.8 kWh/m2 in partial normal operations in Year 2, while the indoor illuminance level meets occupants' requirements. Combined with other relatively constant loads and the reduction of plug and other loads due to COVID building operation restrictions, the total energy use intensity is thereby reduced from 54.1 kWh/m2 to 42.8 kWh/m2, where 5.4 kWh/m2 of energy reduction for Year 2 is estimated to be contributed by the energy-efficient interventions. PV generation is 36.1 kWh/m2, with an increase of 1.4 kWh/m2 from a new inverter. In summary, this paper demonstrates the use of DiBEM through a detailed case study and long-term monitoring data as evidence to achieve ultra-low energy operations. © 2022 Elsevier B.V.

ABSTRACT

The link between the supply and demand sides of manufacturing has become increasingly frail as a result of the COVID-19 outbreak. In this paper, we analyze the key path nodes and propose a closed-loop path value-added strategy for logistics services to optimize the path cost on the supply and demand side under the influence of the COVID-19 epidemic. First, the k shortest path algorithm determines the optional paths in accordance with the structure of the road network made up of all path nodes. Second, closed-loop transportation routes for both forward and reverse transit are constructed using the optional paths. Finally, the transportation service strategy with the optimal choice of transportation cost and transportation time under a multi-stage epidemic is obtained. The method can provide a reference for logistics services. © 2022 IEEE.

ABSTRACT

Background: Shengmai decoction, which has been included in the diagnosis and treatment of coronavirus disease 2019 (COVID-19), is effective in the early treatment of patients with severe COVID-19. Yiqi Fumai lyophilized injection (YQFM) is a modern Chinese medicine preparation of the Shengmai decoction. The mechanism of its intervention at the molecular level in the severe stage of COVID-19 remains unclear. Therefore, it is necessary to investigate the mechanism of YQFM in the treatment of patients with severe COVID-19. Methods: The corresponding target genes of the main active ingredients in YQFM and COVID-19 were obtained by using multiple databases and literature retrieval. A protein-protein interaction network was constructed, and enrichment analysis of the target was performed using Cytoscape 3.8.1. Lastly, the docking of all the identified compounds with angiotensin-converting enzyme II was confirmed by applying molecular docking technology. Results: YQFM has anti-inflammatory effects on RAW267.4 macrophages. The main active compounds of YQFM are all effective anti-inflammatory agents, and these active compounds also show beneficial physiological functions, such as anti-oxidation, anti-bacterial, and anticancer activities. Gene Ontology analysis showed enrichment in the following pathways: lipopolysaccharides, interleukins, NF-kappa B, interleukin-2 and others, revealing that YQFM may play a role in the treatment of patients with severe COVID-19 through these pathways. Conclusion: YQFM has multicomponent and multitarget characteristics, and it could reduce lung injury by inhibiting inflammatory reactions, promoting antiviral activities, and regulating immunity, among other functions, to treat patients with severe COVID-19.

ABSTRACT

Since the outburst of COVID-19, the medical system has been facing great challenges due to the explosive growth in detection and treatment needs within a short period. To improve the working efficiency of doctors, an improved EfficientNet model of Convolutional Neural Network (CNN) was proposed and applied for the diagnosis of pneumonia cases and the classification of relevant images in the present study. First, the acquired images of pneumonia cases were divided to determine the zones with target features, and image size was limited to improve the training speed of the network. Meanwhile, reinforcement learning was performed to the input dataset to further improve the training effect of the model. Second, the preprocessed images were inputted into the improved EfficientNet-B4 model. The depth and width of the model, as well as the resolution of the input images, were determined by optimizing the combination coefficient. On this basis, the model was scaled, and then its ability in extracting the features of deep-layer images was strengthened by introducing the attention mechanism. Third, the learning rate was adjusted by using the Adaptive Momentum (ADAM), and the training efficiency of the model was accelerated. Finally, the test set was inputted into the trained model. Results demonstrate that the improved model could detect 98% of patients with pneumonia and 97% of patients without pneumonia. The accuracy rate, precision rate, and sensitivity of the model were generally improved. Lastly, the training and test results of VGGNet, SqueezeNet-Elus, SqueezeNet-Relu, and the improved EfficientNet-B4 models were compared and evaluated. The improved EfficientNet-B4 model achieved the highest comprehensive accuracy rate, reaching 92.95%. ABSTRACT

Objective This multicenter clinical evaluation analyzed the clinical performance of five fast nucleic acid detection systems for 2019-nCoV. Methods Clinical performance of the five fast nucleic acid detection reagents approved in China was evaluated in the present study. Fifty-seven throat swabs samples from COVID-19 patients and fifteen throat swabs samples from healthy people were collected from the First Affiliated Hospital of Zhejiang University school of Medicine, Tongji Hospital of Tongji Medical College of HUST, and National Institute of Viral Disease Control and Prevention of CDC to evaluate the positive coincidence rate, negative coincidence rate, total coincidence rate, the detection time and retest rate as well as the relation between positive intensity and positive coincidence rate of the five fast nucleic acid detection systems in November 2020. Results The positive coincidence rates of the five kits were 92.59% (50/54), 83.64% (46/55), 98.25% (56/57), 94.44% (51/54) and 98.18% (54/55);and the negative coincidence rates were 93.33% (14/15), 93.33% (14/15), 86.67% (13/15), 100% (14/14) and 93.33% (14/15);and the total coincidence rates were 92.75% (64/69), 85.71% (60/70), 95.83% (69/72), 94.20% (65/69) and 97.14% (68/70), respectively. The positive coincidence rate of the five kits reached 100% for the strong-positive (90/90) and medium-positive samples (84/84), but only 82.18% (83/101) for weak-positive samples (cycle threshold value>33), and the retest rate of two kits were 15.28% (11/72) and 12.50% (9/72), which were both higher than 10%. Total time from sample extraction to amplification was between 32.33-65.33 minutes for these five kits. Conclusion The five fast nucleic acid detection reagents have good performance and can be used as a supplement to routine nucleic acid detection reagents. © 2022 Chinese Journal of Laboratory Medicine. When comparing GINA step at baseline, a higher proportion of patients in GINA steps 4 and step 5 experienced a severe exacerbation compared with patients in GINA steps 1/2, step 2 and step 3 throughout the observation period.
